While your records belong to LMG Family Practice PC, you have the right to know what is in them. If you need a copy of your medical or financial records please ask us.
The primary use for your medical record is for your health care provider to keep track of your personal health history, your family health history, examinations, test results, diagnoses, treatments given, and medications prescribed. We might use your medical records in the following ways:
- The records might be sent to another doctor or therapist who we are asking to take part in your care. In this case, we ask your to sign a consent form that allows us to send that part of your file that relates directly to the condition for which we refer.
- We might use the records as part of our quality assessment program in which we review the care our patients receive to be sure that we are documenting well and that each provider is rendering high quality care.
- Our billing clerks might read a progress note to decide how to enter codes into a bill.
- We might send a copy of a course of your record to an insurer to document a bill we have sent them for your care. We ask you to authorize this by signing a form when you first became out patient.
- Sometimes the law requires us to divulge medical records
as in domestic abuse, malpractice case, or criminal cases.
- Your records might also be seen by business associates whom we hire to do such things as help us devise better record systems, store our records electronically, train our billing staff or other jobs related to efficient operation of the office. To protect your privacy in these cases, we require all of our business associates to comply with our confidentiality requirements and to sign an agreement that limits what they can do with your records.
- Insurers sometimes ask for copies of payments for a certain services to support the billing.
- Our accounts receivable department might need to bill you or refund to your account.
Our staff is trained to protect the privacy of your medical and financial records. They understand that breaches of confidentiality may be grounds for termination from their jobs. If you have any reason to believe that a member of our staff has violated this policy, please countatc usso that we can act appropriately.
